Transition
This is just the same for those with learning difficulties and autism. However, a greater level of support and planning is needed to ensure their future path runs smoothly.
And that's where we can help…. We provide the specialist support needed by young people with learning difficulties and autism as they move into adulthood; a phase often referred to as transition.
Our Transition Service supports those aged between 14 and 25, helping them to get the most out of life and giving them the chance to follow the path that they want.
The transition programme is designed to help young people increase their independence and further develop their personal life skills, based upon the principles of citizenship. These skills will equip them to lead a more fulfilled life in the future and enable them, with support, to direct how they want to live their lives. The service will also help the young people find a suitable permanent home and, where possible, supported living placements. The content of the programme is tailored to each individual and delivered by a skilled team of people. It can include:
- skills for daily living
- vocational training and work experience opportunities
- information and communication technology
- recreational and leisure pursuits
- transitional planning for the next stage of their life
- self-advocacy skills
- communication skills
- safe experimentation and risk taking
- multi-sensory opportunities
- travel training.
