If you are going to be supported by Dimensions we will help you choose a Support Worker you are happy with.

We will support you to do as much as you can yourself. This might be doing things like going out, going shopping or making dinner.

We know everyone is different and we want to make sure that your support is right for you.

We do this by helping you choose what you want to do and how you want to be supported. This is called being person-centred.

We make sure you are getting the right support by holding person-centred reviews with you.

A person-centred review is about what you think about your life and support.

We will talk to you and people who are important to you to find out what you think about your support.

If there is something you are not happy with we will work with you to change it.

We will also write a support plan with you.

Your support plan will say how much money you have to spend on support.

Your support plan will also say what we need to do to support you well.

We will also use a One Page Profile to make sure you get the right support. A One Page Profile is a page that has important information about you on it.

Staff must support you with your money in way that you are happy with.

Lots of people take medication at some point in their lives. You should be able to take your medication with as much support as you need.

You can also make a plan for what you want to happen before you die.
