Hayward’s Avenue Team wins National Learning Disability and Autism Award for transforming lives through personalised support

In 2022, Jack – a young man with a learning disability and a rare condition known as CHAMP1 disorder moved into a specially designed home. But this wasn’t just about new surroundings. It marked the beginning of a life-changing journey made possible by an extraordinary team committed to personalised, co-produced support.

Understanding Jack’s unique needs

Jack’s needs are complex and ongoing. CHAMP1 disorder brings additional challenges, but what makes Jack’s journey so remarkable is the people who surround him. His success is the result of consistent, collaborative effort from a dedicated group who are there for him every single day.

Co-production at the heart of support

At the core of Jack’s support is co-production – a collaborative model where everyone involved in his life plans and acts together. His family, support team, local authority professionals, health specialists, and the Dimensions clinical team all work in unison to give Jack the best possible life. This joined-up approach ensures every decision reflects Jack’s needs, personality, and preferences.

Designing a home that truly fits

Jack’s new home wasn’t a quick fix – it took two years of careful planning. His family played a vital role: choosing the right property, shaping the design, and making sure Jack’s voice was front and centre. The result is a space where Jack doesn’t just live – he belongs.

Although only two people were able to attend the award ceremony, Jack’s journey has been shaped by a much wider team of dedicated individuals who have worked tirelessly to support him every step of the way. These include Ashley Weeks (Locality Manager), Anola Hennessy (Assistant Locality Manager), Julie Bradley, Rashid Djeebet, Dominic Nichols, Lukasz Ablam, Robin Fitzgerald, Fidelis Ughanze, Lisa Puckett, and Luke Scicluna – all of whom have played a vital role in ensuring Jack’s life is the best it can be.

Weekly planning meetings with the team manager, contributions to his support plan and risk assessments, attending health appointments, and being present on difficult days all reflect their continued commitment.

In just three years, Jack has grown in confidence and independence. He’s made friends, joined local groups, and gone on two holidays – trips he helped plan himself. From travelling on a ferry to embracing new routines, Jack is shaping a life filled with choice and meaning.

It is no surprise that the Hayward’s Avenue team were recognised nationally, winning the National Learning Disability and Autism Award for Great Autism Practice. This accolade celebrates the exceptional way the team has supported Jack to live a full, vibrant, and self-directed life.

The award was presented by Peter Andre, adding an extra sparkle to an already emotional and proud moment for everyone involved.

Jennifer began her journey in health and social care in 2012, leaving behind a successful career in sales management to pursue a role that truly made a difference. Starting as a support worker, she brought with her a deep commitment to people and a determination to lead by example.

As a Registered Manager with Dimensions in West Dorset, Jennifer leads teams that support individuals with complex health needs and profound and multiple learning disabilities. She is renowned for her hands-on approach, regularly stepping into direct support roles to maintain close connections with the people her team supports.

Tireless advocate for the people we support

Jennifer is a powerful voice for the rights and choices of those she supports. She consistently ensures that the aspirations of the people in her care inform decision-making, engaging in challenging yet respectful dialogue with families, professionals, and teams as needed.

Known for her unwavering support of her team, Jennifer not only equips them with the skills they need through extra training and learning but also nurtures their emotional wellbeing. Team members often speak of her kindness during personal hardships, while families express complete confidence in her devotion to their loved ones.

Beyond her role, Jennifer leads community fundraising efforts, including events for cancer charities. From organising coffee mornings to leading a skipping competition with over 20 participants, her energy brings together people we support, their families, and colleagues from other providers.
