We’ll work closely with the hospital you’re in, to get to know you well before you leave, to set up the right support with the right housing for you, and to help you have confidence in us, too.
And we’ll work closely with the organisations that pay for your support, to get it right from the start. We’ll ask them to plan for the long term. That sometimes means higher costs and more support to start with, but as you get on with your life the amount of support you need often drops away.
